The objective of the Capital Area Crisis Cooperative (CACC) is to improve the health of persons with MH/SA issues by providing crisis care in more timely and appropriate settings. The goals of the CACC are to:
- Establish standardized MH/SA assessment measures and procedures for all healthcare providers in Wake County
- Develop and support a common database of MH/SA crisis resources
- Develop and support a client tracking system; monitoring compliance and outcomes
- Reduce the number of people with untreated mental health and/or substance abuse conditions
- Ensure an appropriate level of mental health and substance abuse care for those persons presenting at acute hospital emergency departments
- Increase the continuity of care with use of bridging services and direct transfers
- Optimize the community’s capacity for mental health and substance abuse services in the county
- Increase education and awareness for providers and families
The CACC was formed in 2007 and consists of representatives from Duke Raleigh Hospital, Rex Healthcare, Wake County Human Services (the Local Management Entity or LME), WakeMed and client advocates. The CACC is funded by The Duke Endowment and the partner organizations. Position Description: The Project Manager will coordinate the activities of the CACC to ensure the successful implementation of the project including serving as staff to various committees. In addition, the Project Manager will provide strategic planning, identify resources and best practices and facilitate group processes; Define desired results and measurements to assess progress and success; Collaborate with other programs and community partners to accomplish outcomes; Prioritize tasks and coordinate team members to ensure timely completion of tasks; Provide updates (oral/written) on the project; Research, plan, develop and communicate program goals and objectives; Ensure program compliance with local, state and federal guidelines and regulations; Monitor expenditures, contracts and deadlines; Serve on various committees and represent the Cooperative to monitor, develop and respond creatively to meet the changing needs and dynamic circumstances of local mental health reform. The Project Manager position is a time-limited full-time position for one year, with the possibility of an extension.
